The APP will coordinate care of this patient population before and after hospitalization Independently evaluates patients in the clinic for initial and return visits as well as pre & post hospitalization appointments. Ordering/interpreting testing and lab results, modifying plan of care to address results. Provide clinical expertise and direction to support patient medication management. Collaborate with other care providers/consultants to coordinate and communicate clinical care. Maintain accurate documentation regarding patient status, evaluations, and treatments to physicians, streamline the patient transfer process between levels of care, including communicating specific patient needs to internal and external UMHS team members and other ambulatory care providers Perform other related duties as required to assure patient safety and satisfaction. Participate in the development and presentation of educational experiences for patients, families, and health care professionals. Utilize Service Excellence and Patient Family Centered Care in daily practice. Abide by the standards of care and standards of practice established by University of Michigan Health System Collaborate with physicians and care team to provide care across the continuum Practice within the regulations of the Michigan Public Health Code Participate in quality assurance activities Nursing Specific Info Employment is contingent upon successful completion of the credentialing and privileging process. Required Qualifications* NP: Current licensure to practice as a Registered Nurse and Nurse Practitioner specialty certification in the State of Michigan. Current specialty certification to practice as an Adult-Gerontology Acute Care/Acute Care or Primary Care NP/Adult NP Graduate of an accredited Nurse Practitioner program. Certification as a Nurse Practitioner recognized by the National Council of State Boards of Nursing (NCSBN). Current DEA registration required. 1 year recent (within the last 5 years) APP experience working exclusively in Cardiology. Current BLS and ACLS certification PA: Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or one of its predecessor agencies, CAHEA or CAAHEP. Certification by the National Commission on Certification for Physician Assistants (NCCPA). Current license to practice as a Physician Assistant in the State of Michigan. State of Michigan Controlled Substance License Required Current DEA registration required. 1 year recent (within the last 5 years) APP experience working exclusively in Cardiology. Current BLS and ACLS certification Desired Qualifications* 3-5 years recent (within the last 5 years) APP experience treating acute and chronic general cardiology conditions, including but not limited to HTN, HLD, CAD, HFrEF, HFpEF 1 year recent (within the last 5 years) APP experience in ambulatory care 2 years recent (within the last 5 years) full-time APP experience One or more of the following certifications in cardiology: CCKE, CHFN-K, HF-CERT, ABCL, or CHC Interprets telemetry and basic ECG. Interpretation of cardiac catheterizations, echo reports, nuclear medicine studies, rhythm monitoring (Holter?s, ZIO patches, etc.). Demonstrated knowledge of treatment plans for general cardiology conditions. Demonstrated knowledge of adult cardiac physiology. Demonstrated ability to complete history and physical examination; order appropriate diagnostic studies; synthesize information and make informed clinical decisions; develop appropriate care plans; responsive to patient and family preferences and needs. Must demonstrate a high level of independent problem solving, decision-making abilities, and critical thinking skills. Demonstrated ability to collaborate effectively in a multidisciplinary team as evidence by previous experience within the last 2 years. Demonstrates clinical leadership skills: chair a committee, actively involved in professional organizations, developed protocols, quality improvement, etc. Demonstrated excellent, professional verbal and written communication skills. Demonstrated knowledge of treatment plans for general cardiology conditions. Demonstrated knowledge of adult cardiac physiology. Work Schedule Hours/Week: 40 hours Shift: Varies, starting during day, Monday through Friday, but could extend to afterhours or weekends as service expands. Location: Location varies, including but not limited to Cardiovascular Center, Chelsea, Domino Farms Cardiology, Briarwood Cardiology, Brighton Center for Specialty Cardiology, Northville Health Center, and any new locations Cardiology service is added.
