Who We Are:We are a nephrologist-led renal care company helping patients in post-acute settings recover and return home after their hospital stay. We operate primarily in skilled nursing facilities (SNFs), working as an extension of the in-house nursing team and in collaboration with other consultants including cardiology and pulmonology. We aim to offer our partner homes a team-based, renal-focused layer of clinical oversight to ensure patients do not experience preventable renal complications. Visit www.pars-health.com for more information.The Role:We are seeking a full-time nurse practitioner or physician assistant in the Chicagoland area to visit our patients in SNFs. The role will entail visiting ~20-25 patients a day, typically across several nursing homes. You will work in close consultation with our nephrologist leadership to:Conduct physical exams, check vitals, etc.Review labs (H&Ps, BMPs, CBCs, etc.), monitor changes and interpret resultsConsult with other providers, especially for comorbid patientsManage medications (drug interactions, reduce excessive use, etc.)Manage nutrition (fluid / electrolyte balance, etc.)Routinely review patient panels to flag those requiring renal careAttend interdisciplinary meetings with SNF nursing teamTrain homexe2x80x99s nursing staff on renal care basics and when to escalate issuesAnswer questions and provide support to patients and their familiesDocument visits into EMRsManage overall care to prevent Acute Kidney Injury (AKI) and other renal issuesSince we are not proceduralists, our practice is limited to consultative work xe2x80x93 visiting patients, communicating with providers, managing medications, reviewing labs, etc. Most often our impact is driven by making recommendations to the primary service.
