Maintain confidentiality and act in an ethical manner, consistent with professional expectations and the College of Nurses of Ontario (CNO).Retain clinical competence through relevant evidence-based research and legislation.Proficient in computer skills and experience with electronic medical record systems (i.e. PS Suites). Demonstrate ability, commitment to and knowledge of community addictions and mental health care.The Nurse Practitioner will function in a variety of settings: Rapid Access to Addiction Medicine (RAAM) Clinics:Exhibit knowledge of evidence-informed guidelines, legislation, regulation and organizational policies when prescribing controlled drugs/substances (CDS).Demonstrate an understanding of best practices related to the use of CDS and of other pharmacological and non-pharmacological therapeutic options.Excellent communication, decision-making, problem-solving, conflict management and time management skills.Demonstrate ability to work effectively in an interdisciplinary team environment.Health Care Services to CMHA TVAMHS / RAAM clients with no health care provider:Provide health care services to clients 12 years and older that meet eligibility.Provide leadership and guidance for the development of expanded medical services at CMHA TVAMHSPerform comprehensive care using advanced health assessment, physical examination, therapeutic management, consultation, collaboration and referrals in accordance to the Practice Standard: Nurse Practitioner (2018).Work in collaboration with consulting physician and the inter-professional team to provide client-centred health care services to meet the needs of CMHATVAMHS clients.Maintain client records for accuracy and confidentiality. Support the health needs of individuals with a history of addictions, mental health and other chronic illnesses and refer to specialists and other community agencies.Demonstrate ability to deal effectively with sensitive issues with clients and families using a harm-reduction and client-centred approach.Determine the need for follow-up appointments and assist with transfer of care to permanent primary health care services.CWSP / Outreach & Community Partnerships: Work in partnership with Community Withdrawal Support Program (CWSP), Withdrawal Management Services, community agencies and programs.xc2xa0 Integrate CMHA TVAMHS counselling services and support for clients.Coordinate service provision with other health care partners in the development of community care plans and wrap-around services.Assist in program development specific to CWSP that align with best practice standards, evidence-based research and CMHA TVAMHS program mandate.Promote the integration of addiction-based medicine and primary careOffer educational opportunities and consultation as appropriate xc2xa0WHAT YOU’LL NEED TO APPLY:xc2xa0Police Vulnerable Sector Check RequirementsSuccessful candidates must have a Police Vulnerable Sector Check completed in the last six months.
