Responsibilities:Conduct thorough evaluations of patientsxe2x80x99 medical histories, past physical exams and acquired medical documentation.Assess and diagnose acute and/or chronic wounds and order preliminary labs (if necessary).Perform health screening guidelines and assessments per CMS guidelines for MIPS participation.Develop a plan of care and administer comprehensive treatment interventions, such as topical applications, dressing changes, and various debridement techniques.Adhere to evidence-based practices when considering clinical decisions and competencies.Monitor and document patient progress, adjusting treatment plans as needed.Order labs and diagnostic imaging to identify potential barriers to healing.Prescribe wound care medications, wound vacs, and other necessary treatments.Educate patients and caregivers on wound care management and prevention.Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal wound care.Complete physician orders and EMR documentation in line with company policies, procedures, and timelines.Participate in company in-service training and scheduled meetings.Maintain licensure requirements and pursue professional development opportunities.Respond promptly to requests from management and office staff.Communicate any changes in patient status or scheduling issues with office personnel.Willing and able to travel to patientsxe2x80x99 homes or facilities within a designated area.xc2xa0Minimum Requirements:Master of Science in Nursing from an accredited institution.Current RN and APRN licensure from the Nevada Board of Nursing.ANCC/AANP certification in Adult, Family, or Geriatric Medicine.Current licensure from the Nevada Board of Pharmacy and DEA for prescriptions.A minimum of TWO (2) years of wound care experience as a Nurse Practitioner.Experience in home health or hospice care as an RN or APRN.Skilled and confident in sharp debridement techniques and implementing advanced wound management strategies.Actively collaborates with the Director of Clinical Operations on wound progress, incorporating feedback to enhance treatment plans.Holds current AHA BLS and ACLS certifications.Maintains a current TB test (Two-Step, Chest Radiograph, or Quantiferon) as appropriate.Carries current medical malpractice liability insuranceEnsures reliable transportation and maintains current vehicle registration and liability insurance.Ability to read, write and speak fluent English.xc2xa0Highly Desired Certifications (one or more):Adult-Gerontology Primary Care Nurse Practitioner-Board Certified (AGPCNP-BC)xc2xa0Diplomate Certified, Academy of Physicians in Wound Healing (DAPWHc) certificationAmerican Board of Wound Management: CWCA, CWS, or CWSPWound, Ostomy, and Continence Nursing Certification Board: CWCN, CWOCN, COCNNational Alliance of Wound Care and Ostomy: WCC, DWC, OMS, or LLExc2xa0Essential Skills:Excellent communicator with patients, families, and healthcare professionals.Empathetic towards all patient populations.Effective collaborator across multiple healthcare disciplines.Works cooperatively with other providers, clinicians, and office support staff.Adaptable to new wound management strategies and industry trends.Exhibits meticulous attention to detail.Fluency in a foreign language is a plus.xc2xa0Compensation:Annual compensation is based on per patient visit basis,xc2xa0commensurate with experience and nationally recognized wound care specialty certifications.
