COMMUNITY ACCESS NETWORK Psychiatric Nurse Practitioner Chief Medical Officer (CMO) Exempt Under a collaborative agreement with a Community Access Network physician, (if needed), the Psychiatric Nurse Practitioner is primarily responsible for providing quality psychiatric services to adults, including but not limited to diagnosis, treatment, coordination of care, patient education, preventive care and health maintenance. The Psychiatric Nurse Practitioner will collaborate with other members of the Behavioral Health team, primary care providers, nursing staff, pharmacy staff and partner agencies to provider services and incorporate health and wellness into the overall plan of care for patients served. The Psychiatric Nurse Practitioner is expected to be familiar with the philosophy, goals, and objectives of Community Access Network (CAN), a Federally Qualified Health Center Look-Alike, and to work collaboratively to achieve them. The Nurse Practitioner may be directed by the CMO to perform other duties in accordance with funding agenciesxe2x80x99 requirements, contractual agreements, as well as policies, procedures and protocols established by the CAN Board of Directors. o Obtain health history and perform examinations. o Conduct psychiatric assessments o Provide ongoing psychiatric management, including psychopharmacological treatment, psychotherapeutic support, and crisis intervention in collaboration with the treatment and clinical supervisors. o Assure comprehensive healthcare needs are identified and treated o Educate patients about their illness and provide health management information and techniques. o Order and interpret diagnostic tests. Review incoming reports (e.g., lab, x-ray), sign, date and follow-up in a timely manner and in accordance with CAN policies. o Document clinical care and services provided. o Prescribe medication in accordance with Virginia statue, CAN policies, and professional practice guidelines. o Arrange referral for patients requiring services not offered at CAN. xc2xb7 Adhere to the highest standards of medical practice, ethics and professionalism at all times. xc2xb7 Assist the CMO and Chief Medical Quality Officer (CMQO) in establishing medical policies, quality improvement procedures, and evaluating current practices, policies and procedures. xc2xb7 Participate in peer review, quality assurance, and other clinical meetings xc2xb7 Assist the CMO in designing, implementing and evaluating educational programs for CAN patients and clinical staff xc2xb7 Provide direction, training and guidance to the assigned clinical support team. xc2xb7 Recommend changes in all clinical areas, and/or in general areas to improve the patient flow, medical records, billing practices, and appointment practices. xc2xb7 Adhere to UDS quality standards and meet at least the minimum requirement. xc2xb7 Perform their duties as required by the funding agencies to carry out contractual agreements and/or established by CAN management and Board of Directors. o Must become familiar with the Federal and State rules, regulations, and practices in a Community Health Center. o Must become familiar with Federal and State health care programs requirements and assist in carrying out these programs. xc2xb7 Promote the CAN mission statement. xc2xb7 May supervise students in training at CAN, as assigned by the CMO. xc2xb7 Graduate from an accredited school of nursing xc2xb7 Graduate of an accredited Nurse Practitioner Program xc2xb7 Current Advanced Practice Registered Nurse (APRN) Licensure xc2xb7 Must be certified or eligible for certification as an Advanced Psychiatric Nurse Practitioner by the American Nurses Credentialing Center (ANCC) xc2xb7 Current CNOR/CRNFA certification, if applicable xc2xb7 Current D.E.A prescriptive authority xc2xb7 Insurability (malpractice) xc2xb7 Current Cardiopulmonary Resuscitation Certificate (CPR) xc2xb7 Extensive experience with and dedication to serving vulnerable populations xc2xb7 Ability to relate to the public regardless of race, color, creed, religion, gender, gender identification, sexual orientation, national origin, disability, or age The physical requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee is regularly required to sit; use hands to manipulate objects, tools or controls; reach with hands and arms; talk and hear. The employee must frequently lift and/or move up to 5 pounds and occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and the ability to adjust. Noise level in the work environment is usually quiet. CLINICAL HOURS REQUIRED: Provider is required to be available at least 40 hours per week of clinical care assigned to patients; 46 weeks per year, providing a monthly average of 2.0 patient visits per worked hour. It is understood that while CAN is developing its patient panel, the Provider may not reach this productivity level. It is anticipated that the Provider will work approximately 40 hours per week. Provider will provide services at 800 5th Street, Martin Luther King Blvd., Lynchburg VA 24504; or at other CAN sites to meet legitimate business needs. ON-CALL: Primary on-call responsibilities are shared by CANxe2x80x99s primary care provider staff. The Psychiatric Nurse Practitioner will share back-up behavioral health on-call responsibilities with the Psychiatrist.
