My Home Doctor (MHD) is a physician management company providing longitudinal, intensive, patient-centric, in-home care to the most complex and chronically ill patients since 2005. Physicians lead a multidisciplinary team and serve on the frontlines delivering preventive and urgent medical care in the homes of complex, frail, and vulnerable patients. Through the integration of technology, a highly innovative care delivery model, and multidisciplinary clinical teams, MHD delivers health management capabilities at scale for the frailest and most clinically complex segments of the population xe2x80x93 the 3-5% of members that account for 20-30% of all national healthcare expenses annually. As a Physician Assistant (PA), your duties will be to: Provide the full scope of primary care services, which fall under field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care, and health maintenance of their patients. Obtain health history and perform physical examinations. Diagnose and direct counseling for each patient on a plan for treatment. Prescribe medication in accordance with Florida statute and professional practice guidelines. Maintains a problem-oriented health record for each patient seen. Assures continuity of care by providing comprehensive medical services on a timely basis. Provides excellent internal/external customer service via telephone, fax, or face-to-face contact to assist patients with their health care needs. Review incoming reports (e.g. lab, x-ray, EKG) sign, date, and follow-up in a timely manner. Follows established policies, procedures, and objectives; continuous quality improvements objectives and safety, environmental, and/or infection control standards. Qualifications: xc2xb7 Masterxe2x80x99s Degree from accredited PA Program. xc2xb7 Valid State PA license. xc2xb7 Valid DEA License. xc2xb7 Minimum 2-year experience in medical field setting. xc2xb7 BLS and ACLS valid certification. Skill Abilities: Ability to analyze, organize and prioritize work accurately while meeting multiple deadlines. Ability to communicate effectively in both oral and written form. Ability to handle difficult and stressful situations with professional composure. Ability to understand and follow instructions. Ability to exercise sound and independent judgment. Knowledge and skill in use of job appropriate technology and software applications. Ability to understand, respects and accommodates patient’s preferences and needs with regards to their individual beliefs, customs, and practices. Must have ability to provide information clearly, accurately, and effectively to doctors, other nurses, patients, and family members. Must have ability to show compassion, patience and maintain non-judgmental approach to patients and their families. As an APRN, your duties will be to: The Advanced Practice Registered Nurse (APRN) provides a wide range of primary, acute and specialty healthcare services, both autonomously and in partnership with other healthcare professionals, in accordance with State and Federal regulations. Managing patient’s overall care. Prescribing medications and treatments. Ordering and interpreting diagnostic tests, such as x-rays, EKGs, and lab work. Diagnosing and treating chronic and acute conditions, including injuries, infections, diabetes, and high blood pressure. Performing invasive procedures. Counseling. Educating patients on positive health-promoting lifestyle choices and disease, injury, and illness prevention. Qualifications Generally, requires 5 to 7 years of related experience. MSN degree is required. Valid Florida Advanced Practice RN license required. Valid APRN Board certification in specialty area required. NRP and BLS required.
