GrayHawk Health is looking for remote, hybrid, and in-home Nurse Practitioners to join our growing team! Our remote Nurse Practitioners will perform telehealth visits with GrayHawk Health patients. Our in-home Nurse Practitioners will go on the road and provide top primary care to GrayHawk Health patients. Our hybrid Nurse Practitioners will do a little of both! The in-home Nurse Practitioner positions are eligible for quality and production bonuses based on the care that they provide. This position is full-time, Monday through Friday, with no late nights or being on-call. Experience, or lack of it, is not an issue. We put our focus on finding the best people, who happen to also be Nurse Practitioners. We want people who love their community and can relate to our vision of “Empowering all people to live well!”
