Job DescriptionThe Nurse Practitioner focuses on providing general care to patients of all ages and attend to the patientxe2x80x99s needs with utmost patience, care, compassion and understanding. 100% of the position is travel spent going to patient’s residence and providing treatment.Key Responsibilitiesxe2x80xa2 Conduct in-home assessments and evaluation to certify home health care, check vitals, including past medical history, review of symptoms, physical examination and medication review.xe2x80xa2 Obtain consents at the time of visit and submit to intake team within 24 hoursxe2x80xa2 Complete EMR documentation within 24 hours of patient visitxe2x80xa2 Review current condition of patient and determine treatment option and initiate plan of carexe2x80xa2 Facilitate referrals to other healthcare professionals and medical facilities, as neededxe2x80xa2 Write prescription and order for medical tests (as appropriate)xe2x80xa2 Provide patient educationxe2x80xa2 Provides consistent reasonable and prudent practice of advanced nursingxe2x80xa2 Participate in bi-annual ride-along with Medical Director for practitioner and process improvement evaluationxe2x80xa2 Communication between NP and scheduling office will be emailed, signed, or documented for all patients.xe2x80xa2 Discuss any issues or concerns with Medical Directorxe2x80xa2 Dress must be professional, i.e. clean scrubs, clean lab coat, and name tag displayed.xe2x80xa2 Demonstrate promptness, courteousness, and professional demeanorxe2x80xa2 Report patient issues immediately. NPs are not required to be in an atmosphere where they do not feel safe or respected.xe2x80xa2 Annual performance review with Medical Directorxe2x80xa2 Attendance required at Nurse Practitioner provider meetingsRequired Qualificationsxe2x80xa2 Holds current license with Texas Board of Nursingxe2x80xa2 Holds current Basic Life Support certificationxe2x80xa2 Ability to obtain DEA licensurexe2x80xa2 EMR experiencexe2x80xa2 Certification from either the ANCC, AANP or AACN as Adult, Geriatric, Family or Acute Care NPxe2x80xa2 1+ year of clinical experience in highest level of education OR 3+ years of experience as a Registered Nurse in Acute Care, ED, Home Health or Hospice experience.xe2x80xa2 Demonstrated ability to communicate, collaborate, problem-solve, organize, and planxe2x80xa2 Ability to build rapport with patientsxe2x80xa2 Excellent written and verbal communication skillsxe2x80xa2 Ability to spend at least 1 hour with patient in their home, which may be in remote areasxe2x80xa2 Comply with all HIPAA regulations and maintain security of protected health information (PHI)xe2x80xa2 Ability to lift 20 pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurationsxe2x80xa2 Driverxe2x80x99s license required, good driving record; transportation coordinated after approximately 90 days.Preferred Qualificationsxe2x80xa2 ACLS certificationxe2x80xa2 Experience in Gerontology, Cardiology, Internal Medicine and/or Endocrinologyxe2x80xa2 Home care or home visit experiencexe2x80xa2 Excellent administrative and organizational skills and the ability to effectively communicate with patients and their family members (as appropriate)xe2x80xa2 Computer literate and able to navigate using Global Positioning System (GPS)
