Nurse Practitioner – Risk Adjustment

Job Title: Nurse Practitioner xe2x80x93 Risk AdjustmentLocation:Hoag Hospital and its affiliates, CaliforniaResponsibilities:Provide multidisciplinary patient care from admission through discharge.Conduct history and physical exams, diagnose, and treat medical problems.Order and interpret diagnostic and laboratory tests.Perform minor procedures and facilitate the discharge process.Identify and execute Annual Wellness Visits (AWV) and Annual Medicare Advantage Visits (AMAV) for Medicare patients.Manage care for patients with chronic illnesses, particularly in the adult and geriatric populations.Demonstrate advanced knowledge in Risk Adjustment programs and HCC coding and documentation guidelines.Skills:Ability to work with a diverse group of people.Competence in relevant technology, tools, and equipment.Strong clinical knowledge and understanding of applicable procedures.Commitment to patient safety.Ability to travel locally between organizational locations.Qualifications:Required:Masterxe2x80x99s degree in Nursing.Advanced clinical knowledge and skills in patient care and management.Proficiency in Microsoft Office products.Current RN/NP license in California, furnishing license, and DEA number.BLS and ACLS certification.National board certification as a Nurse Practitioner in the area of specialty.Preferred:Two years of recent clinical experience as a Nurse Practitioner in ambulatory and acute healthcare settings.Bilingual or multilingual candidates.
