Palliative Nurse Practitioner — Home Health Setting

We are looking for a Full-time Nurse Practitioner (NP) for a very flexible position as a Palliative Care Nurse Practitioner in the home setting. The Nurse Practitioner must have at least 2 years of nursing experience as a Nurse Practitioner/Advanced Practice Nurse. Experience in hospice and/or palliative care is preferred but not required. The Nurse Practitioner is responsible for the provision of skilled nursing assessment, planning and provision of patient care in order to maximize the palliation of symptoms and determining eligibility for care. In addition, the Nurse Practitioner is responsible for identifying patient or family needs and providing supportive care and appropriate service in accordance with the attending physicianxe2x80x99s orders, the interdisciplinary team plan of care and the policies and procedures of the agency.
