Note: This position will be split between our 102 Main Street, Greenfield, MA location, and our 119 New Athol Rd, Orange, MA location. POSITION TITLE: Advanced Practice Psychiatric Nurse JOB DESCRIPTION: Provides psychopharmacological consultation to primary care and behavioral health staff and provides psychiatric care to patients in an out-patient primary care setting. POSITION SUMMARY: APRN works as part of the patientxe2x80x99s primary care treatment team and provides support in managing the psychological aspects of the patientxe2x80x99s care. REPORTS TO: Behavioral Health Director with shared oversight of medicalpractice by the Chief Medical Officer FLSA STATUS: Exempt PRIMARY RESPONSIBILITIES: Support each primary care team with psychopharmacological perspective on patients Carry a clinical caseload and provide direct clinical services to health center patients (e.g. psychopharmacological consultation, diagnostic evaluation, and medication management) Participate in daily staff xe2x80x98huddlexe2x80x99 and xe2x80x9cwarm hand-offxe2x80x9d Supervise other staff and trainees as needed Attend primary care treatment team meetings Attend weekly consultation meetings with BH Director Participate in staff training around motivational interviewing, behavioral management of complex medical and psychologically impacted patients Complete comprehensive documentation for each patient visit in the patientxe2x80x99s health record Demonstrate a commitment to integrated health care by at least quarterly attendance at oral health department meetings REQUIRED SKILLS, EDUCATION & EXPERIENCE: Strong commitment to CHCFC Mission. Strong interpersonal and communication skills. Ability to work with a diverse population and sensitivity to low-income populations. Registered Nurse licensure in MA Advanced Practice Registered Nurse licensure in MA Current health psychology licensure in MA Five years of related health care experience PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
