As a Primary Care Nurse Practitioner, you will play a crucial role on an interdisciplinary team (IDT), providing direct medical care to participants in the Program of All-Inclusive Care for the Elderly (PACE). You will collaborate with the IDT to develop and implement care plans that enhance health outcomes, meeting participants in various settings including the center, clinic, home, virtually, or external care sites.Key Responsibilities:Provide direct medical care to PACE participants.Complete assessments and contribute to care planning.Manage a panel of participants, ensuring comprehensive care.Initiate, evaluate, review, and modify orders for medications, diagnostic studies, and treatments with appropriate approvals.Daily assessment, initiation, and coordination of participant care under supervision.Document all participant interactions, assessments, and next steps in the EMR within specified timeframes.Make referrals to specialists and request consults as needed.Conduct history and physical exams, and provide participant education.Order, interpret, and follow up on lab tests and results.Perform selected medical procedures.Recognize and escalate abnormalities as needed.Collaborate with team members, including behavioral health and pharmacy, to ensure cohesive participant care.Evaluate and treat participants in various settings, including the center, home, virtually, and skilled nursing facilities (SNF).Facilitate advanced planning discussions with participants.Document chronic conditions via HCC capture.Qualifications:Masterxe2x80x99s degree or higher in Nursing.Valid Nurse Practitioner (NP) license in the state of California (CA).Valid American Heart Association Basic Life Support (AHA BLS) certification.Minimum of 4 years of experience as a Nurse Practitioner, with experience in any of the following:Geriatric CareGeriatric/LTC (Long-Term Care)Bilingual in Spanish.Reside in or willing to commute daily to Los Angeles, CA.Seeking a full-time position.Must not have applied or worked for myPlace Health in the last 12 months.Must have discussed this role with your relode recruiter via phone or video.Preferred Qualifications:Experience working with the PACE model (Program of All-Inclusive Care for the Elderly).Schedule and Shift Details:40 hours per week, no on-call.
