Job Title: Nurse Practitioner xe2x80x93 Risk AdjustmentLocation: Costa Mesa, CADuration: 4 Months( possible extension)Pay range: $70 – $75/hr. on W2 (Negotiable)Primary Duties and Responsibilities:The Nurse Practitioner is responsible for assisting in the expedient multidisciplinary care of patients for Client Hospital and its affiliates, coordinating seamless patient care from admission through discharge. Providing supplemental medical care to patients with a focus on quality and patient experience, the Nurse Practitioner can perform history and physical exams, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests, performs minor procedures and facilities the discharge process. Client Clinic Primary Care/Internal Medicine:Responsible for identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and Annual Medicare Advantage Visits (AMAV) and execution of guided processes to complete AWV and AMAV. Works with patients with chronic illnesses commonly encountered in the adult and geriatric patient population. Responsible for assessment, diagnosis, education, health promotion, and care coordination for patients with acute and chronic health needs. In addition, shows advanced knowledge in Risk Adjustment programs and HCC coding and documentation guidelines.Diversity xe2x80x93 Demonstrates ability to work with a diverse group of people in a manner that enables them to reach their full potential, in pursuit of organizational objectives.Technology & Equipment Skills :Maintains knowledge of technology, tools, equipment, and other devices in work area. For clinical position this includes equipment and devices pertinent to treatment, procedures, and practice.Job Knowledge :Demonstrates full knowledge and understanding of concepts and procedures needed to be successful in the role, including knowledge of applicable department, organization and external rules and regulations impacting day to day processes. For clinical positions, this includes clinical procedures required by department and unit, as demonstrated by unit-based competency testing Patient Safety xe2x80x93 Embraces the principle of keeping patients safe by following safety policies and procedures, treating people with respect, and speaking up in Patient Safety related situations.
